Möbius syndrome(en : Moebius syndrome)
在兒科看到了一個Möbius syndrome的小孩。這是一個先天性顏面神經核與臉部肌肉發育不全。主要症狀表現有
- 臉部雙癱
- 單獨/雙側外展麻痺/涉及多個腦神經
- 小顎畸形和小口畸形
- 可能有顎裂和牙齒異常
- 先天畸形
- 智力障礙

目前認為與這個疾病有關的
- Möbius syndrome通常一訂有先天性臉部雙癱
- 大多是偶發的
- 與基因有關的話，已確定的基因座位於 13、3 和 10 號染色體上
- 家族性遺傳模式是：AD、AR、X連鎖
- MRI：r/o 腦畸形
- EMG：確定受傷時間
- r/o 子宮內毒素、血管畸形或梗塞(懷孕低血壓造成第六第七對腦神經血液灌流不足發育不佳)
▲附上第六對(灰)與第七對(黃)腦神經的解剖位置，十分接近，所以懷孕低血壓容易導致兩個一起發育不好，所以哭泣時由於depressor anguli muscle沒有發展好，所以嘴角會往下拉▼
Depressor anguli oris muscle aplasia
剛剛提到這個肌肉有受損，真的要診斷的話要用Electrodiagnostic studies。
但通常不需要，可能與外傷做differentiate
結果可以看到
- Normal conduction velocity and latency of the facial nerve
- Fibrillations (-)
- Absent or decreased motor unit potentials
大約 10% 的病例與其他更重要的先天性異常有關，尤其是心血管系統（Cayler cardio-facial syndrome）
Birth injury
先天原因的部分多是較大的足月新生兒，經過長時間分娩後陰道分娩(Large term newborn, delivered vaginally after a prolonged labor)
完全性單側面癱可能是輕微的(Complete unilateral facial palsy can be subtle)
臨床表現(Clinical features)
- Failure of eye closure
- Flaccid paralysis become obvious when the child cries
- The normal side appears paralyzed
- When paralysis of the facial nerve is partial, the orbicularis oculi is the muscle most frequently spared
